長者社區照顧服務券計劃-Community care service voucher for the elderly

• 「錢跟人走」：長者可按個人需要，使用社區券選擇合適的社區照顧服務。
• 「能者多付」：長者及同住家人負擔能力越低，政府的資助越多。
• 多元化的服務選擇：長者可於一個曆月，向不多於2間認可服務單位選擇社區券面值內的服務項目及服務組合。

• "Money-following-the-user": directly subsidise eligible elderly persons so that they can freely choose and switch Recognised Service Providers (RSPs) under the Scheme, and adjust service items and volumes.
• "Affordable users pay": the less an elderly person can afford, the more government subsidy he/she receives.
• Mixed mode services: elderly persons can use both day care and home care services at the same time.

以「能者多付」為原則，長者及同住家人的負擔能力越低，政府的資助越多。

按長者的家庭住戶收入進行經濟狀況審查，定有六個共同付款級別，分別為服務組合價值的5%、8%、12%、16%、25%及40%。

公共福利金，包括長者生活津貼、高齡津貼、傷殘津貼等，不計作家庭收入。

如申請人選擇不透露家庭收入，便須繳付最高共同付款金額(即服務組合價值的40%)，而長者家庭住戶的資產並不計算在內。

 SWD will take into account the family household income of the elderly person and his/her residing family members to conduct financial condition assessment with reference to the quarterly Median Monthly Domestic Household Income released by the Census and Statistics Department so as to determine the co-payment category of the elderly person. There are six categories of co-payment. Elderly persons are required to pay for the co-payment amount according to the rate of 5%, 8%, 12%, 16%, 25% or 40% of the service package value of CCSV while the government will pay for the remaining amount of the service package value. 

在「安老服務統一評估機制」下獲評定及建議為適合社區照顧服務或院舍照顧服務，並正在資助長期護理服務中央輪候冊(中央輪候冊)輪候資助社區照顧服務及/或院舍照顧服務，而尚未接受任何院舍照顧服務或資助社區照顧服務的長者。

 Eligible elderly persons who have been assessed and recommended for CCS or residential care services (RCS) under SWD's Standardized Care Need Assessment Mechanism for Elderly Services and are waitlisting for subsidized CCS and/or RCS on the Central Waiting List (CWL) for Subsidized Long Term Care Services without any kind of RCS or subsidized CCS being received. 

1. 提出申請: 長者或其家人向醫務社會服務部、所屬地區的綜合家庭服務中心或長者服務單位的負責工作員提出申請受資助的長期護理服務。   
2. 初步甄別: 負責工作員進行初步甄別，並按需要轉介長者接受統一評估。   
3. 進行評估: 評估員透過家訪及會面為長者進行評估。   
4. 解釋評估結果: 負責工作員向長者解釋評估結果及服務選擇。   
5. 制定照顧計劃: 負責工作員協助長者制定照顧計劃，並申請適切的服務。

SWD will send invitation letters to the eligible elderly persons in batches according to their application dates on the CWL for subsidised LTC services, inviting them to join the Scheme. The eligible elderly persons who are invited can submit the completed application form together with the supporting documents to CCSV Office. They can also submit the online application via VISE. 

In general, the SWD will complete the vetting and approval process and issue the vouchers within four weeks upon receipt of applications.
